Person and Whānau Centred Care

Person and whānau centred care is collaborative healthcare focused on meeting the needs, values, and desired outcomes of individuals and whānau.

The model of person and whānau centred care (PWCC) highlights how you can provide care that meets the needs of individuals and their whānau and that improves their health outcomes. You will be able to use the guidance provided to plan your professional development, and enhance your professional practice.

  • Person and whānau centred care poster: This poster presents the PWCC diagram on one side. On the other side, a plain-English version of the PWCC model for consumers and their whānau is presented.
