Person and Whānau Centred Care

Person and whānau centred care is collaborative healthcare focused on meeting the needs, values, and desired outcomes of individuals and whānau.

The model of person and whānau centred care (PWCC) has been developed to support and enhance the physiotherapy profession in Aotearoa New Zealand. It highlights how you can provide care that meets the needs of individuals and their whānau, and that improves their health outcomes. You will be able to use the guidance provided in the model to plan your professional development, and enhance your professional practice.

Download the PWCC document and poster here. The PWCC document contains the full model, and a selection of consumer stories to illustrate what the PWCC behaviours ‘look like’ in practice. The poster presents the PWCC visual image, and a plain-English version of the model which has been written for consumers.
